If you want to simply turn on and off the fan rather than control the speed of the fan, this controller may be a good option. It has two outputs. Output 1 is a relay rated at 1 HP 250 Vac / 1/3 HP 125 Vac (16 A Resistive), output 2 is a relay rated at 3A / 250 Vac.
Each output can have a set point based on temperature or humidity(not both). Depending on your desired control logic you should be able to make this work, although, since the second relay is only rated at 3 amps so, if you intend to use it you may need an intermediate relay which can handle the 4 amp draw from the fan.
